HIV attacks the body’s immune system, specifically the CD4 cells (T cells), which help the immune system fight off infections. Untreated, HIV reduces the number of CD4 cells (T cells) in the body, making the person more likely to get other infections or infection-related cancers.
Antiretroviral therapy should be initiated regardless of CD4 count in pregnant patients, patients with HIV-associated nephropathy, and those with hepatitis B virus (HBV) coinfection when treatment of HBV infection is indicated
In February 2016, Jordon suddenly found himself too weak and tired to attend the community-college classes he had enrolled in; he could hardly lift his head from his mother’s couch. He wasn’t accustomed to being sick and had tested negative for H.I.V. just five months before, so thinking he had a bad cold, he waited weeks before his family forced him to go to the emergency room at a hospital in his small town, where he was tested again. “The doctor said to me, ‘Your H.I.V. is so bad — how could you not know?’ ” Jordon recounted through tears. He ended up in intensive care for three weeks. “I honestly didn’t believe it.” He paused and then added quietly, “It was the worst day of my life.”
A single case report detailed a possible cure resulting from stem-cell transplantation from a CCR5-delta32 homozygous donor (performed to treat acute myelocytic leukemia). Although this important finding is unlikely to impact routine management of HIV infection, it does suggest that reconstitution of a host immune system with a population of mutant cells is a possible avenue of research to explore. 
Federal and state programs are also hampered by policy decisions grounded in ideology rather than science such as the allocation of more than $1 billion to failed abstinence-only sex education programs or the enactment of outdated HIV criminalization statutes. In more than 30 states, people living with HIV can be tried and imprisoned simply because a partner accuses them of withholding their HIV status. There’s no proof these laws work, and they run counter to public health by perpetuating stigma and subsequently deterring people from getting tested or treated for HIV.
Choice of initial and/or second-line therapy for HIV-2 has not yet been defined. HIV-2 appears to be resistant to NNRTIs intrinsically, but may be sensitive to NRTIs, though the mechanism is poorly understood. Protease inhibitors have shown variable effect, while integrase inhibitors are also being evaluated. Combination regimens of the above listed therapies are being looked into as well, also showing variable effect depending on the types of therapies combined. While the mechanisms are not clearly understood for HIV-1 and HIV-2, it is known that they use different pathways and patterns, making the algorithms used to evaluate HIV-1 resistance-associated mutations irrelevant to HIV-2.

Treatment stresses aggressive combination drug therapy for those patients with access to the expensive medications and who tolerate them adequately. The use of these multi-drug therapies has significantly improved and prolonged the life of HIV/AIDS patients in the United States.

HIV can be suppressed by combination ART consisting of 3 or more ARV drugs. ART does not cure HIV infection but suppresses viral replication within a person’s body and allows an individual’s immune system to strengthen and regain the capacity to fight off infections.
Since the Bergalis case, many U.S. dentists, physicians, and with AIDS have begun disclosing their status to their patients. Faya v. Almaraz, 329 Md. 435, 620 A.2d 327 (Md. 1993), illustrates the consequences of not doing so. In Faya, the court held that an HIV-positive doctor has the legal duty to disclose this medical condition to patients and that a failure to inform can lead to a Negligence action, even if the patients have not been infected by the virus. The doctor’s patient did not contract HIV but did suffer emotionally from a fear of having done so. The unanimous decision held that patients can be compensated for their fears. Although this case dealt specifically with doctor-patient relationships, others have concerned a variety of relationships in which the fear of contracting AIDS can be enough for a plaintiff to recover damages.

The CDC reported that, at the end of 2014, the most recent year for which national prevalence statistics are available, there were 955,081 adults and adolescents living with HIV infection in the United States, 521,002 of whom had infection classified as stage 3 (AIDS). 
“It’s no longer a death sentence,” Boswell said of HIV. “It’s a very different time now. Most people just diagnosed with HIV will live an almost normal life span if they get an early diagnosis, appropriate care and stay on their medications.”
If a person has been exposed to the virus, it is crucial that they get tested as soon as possible. The earlier HIV is detected, the more likely the treatment will be successful. A home testing kit can be used as well.
Other tests can detect antibodies in body fluids other than blood, such as saliva, urine, and vaginal secretions. Some of these are designed to be rapid HIV tests that produce results in approximately 20 minutes. These tests have accuracy rates similar to traditional blood tests. OraQuick is an at-home test that uses an oral swab to detect HIV antibodies in oral fluid. Clearview is another rapid HIV test that can detect HIV antibodies in blood or plasma. HIV home-testing kits are available at many local drugstores. Blood is obtained by a finger prick and blotted on a filter strip. Other test kits use saliva or urine. The filter strip is mailed in a protective envelope to a laboratory to be tested. Results are returned by mail within one to two weeks.
The HIV DNA copy is incorporated into the DNA of the infected lymphocyte. The lymphocyte’s own genetic machinery then reproduces (replicates) the HIV. Eventually, the lymphocyte is destroyed. Each infected lymphocyte produces thousands of new viruses, which infect other lymphocytes and destroy them as well. Within a few days or weeks, the blood and genital fluids contain a very large amount of HIV, and the number of CD4+ lymphocytes may be reduced substantially. Because the amount of HIV in blood and genital fluids is so large so soon after HIV infection, newly infected people transmit HIV to other people very easily.
