During viral replication, the integrated DNA provirus is transcribed into RNA, some of which then undergo RNA splicing to produce mature mRNAs. Some religious organizations have claimed that prayer can cure HIV/AIDS. In 2011, the BBC reported that some churches in London were claiming that prayer would cure AIDS, and the Hackney-based Centre for the Study of Sexual Health and HIV reported that several people stopped taking their medication, sometimes on the direct advice of their pastor, leading to a number of deaths.[263] The Synagogue Church Of All Nations advertised an “anointing water” to promote God’s healing, although the group denies advising people to stop taking medication.[263]

Patients beginning ART sometimes deteriorate clinically, even though HIV levels in their blood are suppressed and their CD4 count increases, because of an immune reaction to subclinical opportunistic infections or to residual microbial antigens after successful treatment of opportunistic infections. IRIS usually occurs in the first months of treatment but is occasionally delayed. IRIS can complicate virtually any opportunistic infection and even tumors (eg, Kaposi sarcoma) but is usually self-limited or responds to brief regimens of corticosteroids.

HIV-positive patients who are taking anti-retroviral medications are less likely to transmit the virus. For example, pregnant women who are on treatment at the time of delivery transmit HIV to the infant about 5% of the time, compared to approximately 20% if medications are not used.

The majority of people on HIV treatment in countries like Australia will have long-term suppression of symptoms and a reduced viral load. Without HIV treatment people with HIV may develop AIDS and die from infections, cancers and other illnesses the immune system can no longer fight.

The Centers for Disease Control and Prevention (CDC) recommends opt-out HIV screening for patients in all health-care settings; persons at high risk for HIV infection should be screened at least annually [2]

Protease inhibitors. Protease inhibitors work by disabling protease, an enzyme necessary for HIV reproduction. Protease inhibitors include saquinavir (Invirase), ritonavir (Norvire), indinavir (Crixivan), nelfinavir (Viracept), amprenavir (Agenerase), kaletra, and many others.

Medications are also used to prevent opportunistic infections (such as Pneumocystis carinii pneumonia) and can keep AIDS patients healthier for longer periods of time. Opportunistic infections are treated as they occur.

HIV enters target cells via a multistep process. First, HIV binds to the CD4 receptor, leading to conformational changes in the viral gp120 envelope protein that enable binding to chemokine coreceptors for HIV. Chemokine receptor engagement triggers conformational changes in the HIV gp41 envelope protein, leading to fusion of HIV and the target cell, resulting in delivery to the viral core.
